Fill the appointment form to arrange a meeting with a doctor. We will be pleased to receive you.

Items submitted to the Attendance Office must include following:

• The Hospital Card number or preferably the Card.
• As alternative to the above, your date of last visit.
• Medical history from previous hospital(s) if any.
• Medical Laboratory Test result if any.
• Test sample(s) if requested by Doctor to come with any.
• Registration Fee if visiting for the first time
